Currently it seems that dmcc outputs mono wav files. Is there a way to have it do stereo instead?

search the forums and FAQs for "selective listen hold". while it is possible to get separate streams for parties in a call I suspect you will find it expensive (requires a TSAPI Advanced User license), and complex to implement.

I know good old Nortel CS1000 supported IP phones stream copies of audio to the CS1000.

Similarly, does Avaya Communication manager (6.x or 7.x) support duplicate media streaming (AML) where there is an audio (G.711 uLaw selected as an option) stream for what the agent hears and another for what an agent says?

yes - see Selective Listen Hold/Retrieve in the TSAPI documentation.
