Is it possible to make the skill change done through SMS effective without forcing the agent logout/login?

Only by using the Add/Remove skill feature access code (FAC). You can not add/remove an agent skill programatically through SMS or the SAT without an accompanying agent logout/login.

You can use the Add/Remove skill FAC from a TSAPI, JTAPI or DMCC application, but not SMS.
